Sometimes it works like that sometimes it doesnt.

Also as Biff had happen to him, one of the most common insured item drops is sleeves, I dont know why but they seem to get hit a lot, and ive looted a lot. But yes item insurance bug can hit anything, apparently some people know ways to make all of your items drop when they kill you, but I'm a little sceptical about this.

I-C contains some very unpopular people so I'm sure one of us would have been the victim of this exploit by now if it existed, which is what leads me to believe that it is in fact just a random occurance.

Yeah I returned it to Horny, he wasn't hit by the bug though, he simply forgot to insure. Was totally random I even found it, because I only opened about 5 corpses at the harrower just to steal some pots
